District students :

Please complete the AT-17 form, Application for Employment, attach a physical exam completed within the past 12 months or the completed AT-16 form, Physical Fitness Certification. Documents can be handed in to the health office of your designated building. 

Students who attend private school and reside in the district:
Please complete the application, AT-17 form, attach a physical exam completed within the past 12 months or the completed AT-16 form, and proof of age (ie. birth certificate, learners permit, drivers license). Documents can be brought to the health office of your designated building.
